Clavicle Fractures Flashcards
1
Q
MC location
A
- middle 1/3rd
2
Q
Last ossification center during development?
A
- sternal end of clavicle
- age 22-25
3
Q
What does the medial 1/3rd protect?
A
- brachial plexus
- subclavian & axillary vessels
- superior lung
4
Q
Components of the coracoclavicular (CC) ligament
A
- trapezoid
- conoid
5
Q
Function of CC lig
A
- vertical stability to AC jt
6
Q
MOI
A
- fall onto affected shoulder (MC)
- direct impact
- FOOSH
- M. contraction (i.e. seizures)
- pathologic bone
7
Q
Body positioning during eval
A
- adducted and supported by contralateral arm to unload shoulder
8
Q
Eval components
A
- neurovascular: eval elements lying post. to clavicle
- integumentary: proximal end of fx tents skin –> r/o open fx
- pulm: dim breath sounds –> ? ipsilat PTX 2/2 apical lung injury
9
Q
MC fx a/w clavicle fx
A
- rib
10
Q
What is a/w prox 1/3rd clav fx?
A
- brachial plexus injury 2/2 traction
11
Q
XR views
A
- AP
- 30 deg cephalad (remove overlapping thoracic anatomy)
- apical oblique (for min displaced fx, esp in pedi)
- CXR for comp B
12
Q
Why order CT?
A
- prox 1/3rd fx: differentiate SC disloc v. epiphyseal injury
- dist. 1/3rd fx: eval articular involvement
13
Q
Define Group I
A
- middle 1/3rd fx
14
Q
Define Group II
A
- distal 1/3rd fx, further described by type
15
Q
Group II Type I
A
- min displacement 2/2 inter-ligamentous fx, either b/t conoid & trapezoid or CC & AC
16
Q
Group II Type II
A
- displaced 2/2 fx med to CC lig
17
Q
Group II Type IIA
A
- conoid & trapezoid attached to distal frag
18
Q
Group II Type IIB
A
- conoid torn, trapezoid attached to distal frag
19
Q
Group II Type III
A
- intra-articular fx to ACJt
- no ligamentous injury
20
Q
Group III
A
- prox 1/3rd fx
21
Q
Non-op tx
A
- immob x 4-6w
- AROM elbow, wrist, fingers
- serial XR
22
Q
Op techniques
A
- plate & screws
- IM fixation (Hagie pin, Rockwood pin, Sonomal nail)
23
Q
Complications
A
- NV compromise: uncommon
- malunion
- nonunion: 2/2 severity of initial trx (open), extent of initial displacement of fx frags, soft tissue interposition, refx, inad immob length, primary ORIF
- post-trx arthritis: intra-articular to SC and AC jts
