Criteria for diagnosis of periodontitis
Interdental clinical attachment loss is detected at two/more non-adjacent teeth
OR
Buccal or oral clinical attachment loss is 3/more mm with pocketing of >3mm detectable at 2/more teeth

S a) ITD CAL: 1-2 mm
b) RBL: <15% of coronal third
c) Tooth loss: 0

Severity:
a) ITD CAL: 3-4mm
b) RBL: 15%-33%
c) Tooth loss: 0
Complexity:
a) PD:
State the clinical features of stage III periodontitis
Severity:
a) ITD CAL: >/= 5mm**
b) RBL: extends to middle 3rd & beyond
c) /=6mm
b) Vertical bone loss of >/=3mm**
c) Moderate ridge defects
d) Class II/III furcation involvement
State the clinical features of stage IV periodontitis
Severity:
a) ITD CAL: >/= 5mm
b) RBL: extends to middle 3rd & beyond
c) >/= 5 teeth lost
Complexity:
Needs complex rehabilitation due to:
a) Masticatory dysfunction
b) Secondary occlusal trauma (tooth mobility degree >/= 2)
c) Severe ridge defects
d) Bite collapse, drifting, flaring
e) <20 remaining teeth (10 opposing pairs)
Grade A
- No RBL nor CAL over last 5 years
- <25% bone loss/age
- Heavy biofilm deposits w/ low levels of destruction
- Non-smoker
- Non-diabetic
Grade B
- RBL/CAL <2mm over 5 years
- 25%-100% bone loss/age
- Heavy biofilm deposits w/ proportional destruction
- Smokes <10 cigarettes per day
- HbA1c <7%
Grade C
- RBL/CAL >/= 2mm over 5 years
- > 100% bone loss/age
- Heavy biofilm deposits w/ disproportionately high destruction
- Smokes >/= 10
Grade C
- RBL/CAL >/= 2mm over 5 years
- > 100% bone loss/age
- Heavy biofilm deposits w/ disproportionately high destruction; rapid progression/early onset
- Smokes >/= 10 cigarettes per day
- HbA1c >/=7%
