Q

How does a therapist using Reality Therapy take the mystery and uncertainty out of the therapeutic process when a client says, “I’m depressed. My depression is the problem. Why are you talking about my relationships?”

A

Answer: see pages 343-344 - Procedures that Leads to Change
In the first session a skilled therapist looks for and defines the wants of the client. The therapist also looks for a key unsatisfying present relationship—usually with a spouse, a child, a parent, or an employer. The therapist might ask, “Whose behavior can you control?” This question may need to be asked several times during the next few sessions to deal with the client’s resistance to looking at his or her own behavior.

Q

Behavioral therapy: There are two types of conditioning: classical and operant. Define each and give an example of when you would use them.

A

Classical conditioning - refers to what happens prior to learning that creates a response through pairing. Pavlov - rang bell when gave dog food, so eventually paired bell with food and just bell would elicit salivating. But pairing will diminish if not reinforced ( e.g. using bell without food). systematic desensitization: client imagines successively more anxiety-inducing situations while engaging in a behavior that competes with anxiety.

Operant conditioning: a type of learning in which behaviors are influenced by the consequences that follow them. getting a gift after getting an A on a test. Can also work in a negative way - negative consequences reduces chance of repeating the behavior.

Using cognitive behavioral therapy, explain the three components of the ABC framework and identify these as related to Nicks’ problem.

A

ABC framework is a practice used in REBT and helps us understand the client’s thoughts, feelings, events and behavior. A stands for the existence of a fact or activating event. B stands for a person’s belief about the activating event. C stands for the emotional and behavioral consequences. Related to Nick’s problem:
A - not meeting his desired score on SAT
B - belief that he needs to be perfect academically

Q

Do you think counselors find Reality Therapy to be a good model for counseling clients? Discuss your reasons for why or why not.

A

(page 354-355) -
Advantages - short-term focus, strong focus on client’s self-evaluation, existential underpinnings which put emphasis on client’s responsibility and choice, client-determined direction, can be effective with reluctant and resistant clients.

Limitations - does not provide much emphasis on certain aspects of the counseling process: role of insight, the unconscious, past experiences, therapeutic value of dreams, and transference. Views mental illness as result of a person’s unsatisfying present relationships (and behavioral choices) which may not be appropriate for those suffering from chronic depression or schizophrenia. Can lend itself to a counselor trying to fix problems, especially by inexperience counselors. Uses concrete language and simple concepts, which may cause people to view it as a simple/easy approach, when it actually requires practice, supervision and continued learning by the counselor. )

A

D. If you are at a 10 when you are ready to leave counseling and a 1 when you called to make your first appointment, where are you now?

21
Q

the book outlines 6 propositions/”basic Dimensions of Human Existence” of Existential Therapy. Choose 1, briefly explain it and how you might apply it to a concern in your own life.

A
  1. Capacity for self-awareness - based on freedom, choice & responsibility. self-awareness is at root of other capacities.
  2. Freedom and responsibility - we are free to choose among alternatives and so play a large role in shaping own destiny. Freedom to choose means responsibility for actions or failures to act. need to be authentic.
  3. Striving for Identity and Relationship to others - courage to be ourselves, learning to be alone - and responsible for self, experiencing relationships/relatedness, struggle with our identity - be willing to be/ find own answers.
  4. The Search for Meaning - What’s my purpose? be willing to discard old values, work through meaninglessness, create new meaning (must pursue obliquely). can find meaning in suffering.
  5. Anxiety as a Condition of Living - freedom/responsibility can bring anxiety. Normal is fine, neurotic is not. learn to tolerate ambiguity.
  6. Awareness of Death and Nonbeing - death isn’t a threat, can help us figure out how we want to live.
22
Q

Why aren’t there any “basic techniques” in Person-Centered Therapy?

A

PCT is nondirective, being with the client and entering into their world through empathy is sufficient technique needed for clients to change and grow. if techniques are suggested they need to be done in a way that fosters the empathic relationship between client and therapist. Clients have the necessary resources to grow so a technique is not needed. The therapists’s presence is needed.

23
Q

What is social interest and why is it important?

A

the action line of one’s community feelings and involves being as concerned about others as one is about oneself. Capacity to cooperate and contribute. Allows us to identify with and give empathy to others. Willingness to care for others.

24
Q

Pick one child Adler lists in his birth order analysis and describe the expected traits.

A

Oldest child - can be spoiled if an only child. dependable, hardworking, driven. if younger sibling then can be bossy, driven/high achiever.

Second child (of only two) - may compete with older sibling, may succeed where older one failed to gain achievements/recognition, often opposite to first born.

MIddle child - feels squeezed out, life is unfair, “poor me”, within family conflict may become the peacemaker, If 4 kids, 2nd might be “middle” and 3rd be similar to 1st

youngest child - baby of fam, pampered, may become expert at “helplessness” and getting others to help him. tend to go their own way so as to outshine others.

only child - can be similar to oldest child but may not learn to share/cooperate with others. often pampered, may want center stage, better at dealing with adults.

25
Q

List a strength and limitation of Adlerian therapy.

A

Strength - flexible and integrative - allows for use of variety of relational, cognitive, behavioral, emotive, and experiential techniques. can apply to wide variety of clients

Limitation - limited research, more in past 25 years but not enough empirical testing. Adler focused on teaching/practicing and not organizing/presenting a well-defined theory. Can seem simplistic.

26
Q

A key concept of Adlerian theory is birth order, Explain and relate it to own family dynamic.

A

Birth order looks at relationship between siblings and psychological birth position in family. not deterministic, but probably. is more about person’s interpretation of their place in family. Adler sees most problems as social in nature and relationships with family is one of earliest and maybe most influential.

I am the oldest of 6 - blended family and with age gaps, can feel like 2-3 families. I don’t think I was bossy, but was responsible and cared for younger siblings. generally hardworking and reliable. but also could function as peacemaker. would say I “compete” most with full sibling, not half siblings.

27
Q

why does psychoanalytic theory place huge emphasis on the unconscious?

A

the unconscious is where our memories and experiences are stored including what we repress. as we are generally unaware of most of our psychological functioning as it takes place, it is important to understand the unconscious to see what is driving our behaviors, thoughts, feelings. when we understand what’s underneath, then we can exercise our choice.
