Q

What defines Tangible Hydra-PEG?

A

PERMANENT coating design to improve comfort, made of hydrophilic polymers, 90% water, inc. wettability, lubricity, deposit resistance; DO NOT want to use abrasive or alcohol cleaners for these

Q

4 Major functions of wetting and soaking agents?

A
  1. Maintain lens hydration
  2. Disinfection
  3. Mechanical buffer/cushion between lens and cornea
  4. Temporarily enhance surface wettability

5
Q

For temporarily enhancing surface wettability - what are we doing to achieve this/parameters to achieve this?

A

-We are trying to make it into a hydrophilic end. When we store it dry, it leans toward a hydrophobic end on the surface.
-Ionicity
-And these wetting agents typically contain: Methylcellulose derivatives (viscous) and PVA (polyvinyl alcohol), which are water soluble, non-viscous, non-toxic

Q

Daily Cleaners GP vs SCL?

A

More import in GP wear bc lenses need to last longer and surfaces readily attract oils from eye/eye creams, soaps, makeup remover etc

10
Q

Is rub necessary with Daily Cleaners?

A

Yes, rub 2-3 drops of DC in palm of hand for 15-30 second because digital pressure/friction helps remove deposits (not excessively bc can warp/break lens); clean in palm with L-R motion (not fingers and not circular motion bc can break or change lens parameters)

25
What should the active ingredients in solution preservatives do? (5 things)
1. Disinfect 2. Prevent toxic rxns 3. Avoid adverse effects on lenses 4. Wettability/parameters 5. Enhance tear film compatibility
26
Preservative sensitivity symptoms include? Decrease or increase over time of use?
Dryness, redness/burning, itching, lens awareness. These will increase over time
27
Signs of sensitivities to a solution?
Injection, follicles and papillae, tear film debris, mucus strands, superficial punctate staining
28
Bactericidal preservatives vs Bacteriostatic preservatives?
Kills microorganisms vs Inhibits microorganism growth
29
Preservatives!
Coming note cards
30
Benzalkonium Chloride (BAK)
Quaternary ammonium compound
31
BAK is effective against..? And used in a majority of opthalmic preparations for what type of solutions?
Wide spectrum of bacteria and fungi; GP solutions, is NOT used in SCL solutions due to binding properties that can be toxic
32
BAK effectiveness is enhanced with?
EDTA
33
Chlorhexidine
Bactericidal soln for GP and SCLs (Greater conc in GP; has limited binding capacity w/ GP lenses); it is not used that often in SCL soln
34
Chlorhexidine is relatively ineffective against what? And limited effectiveness in?
Serratia; limited eff in yeast and fungi
35
How is efficacy improved for Chlorhexidine? Addition of _____ or _____
EDTA or Thimerosal
36
Chlorhexidine examples?
B&L Boston Line: Simplus (MPS), Original Conditioning, Advance Conditioning (lower conc than original)
37
Thimerosal
Bactericidal; organic mercurial compound, slow acting but good broad spectrum
38
Thimerosal is ineffective against what (in low conc.)?
Ineffective against Pseudomonas
39
Why is Thimerosal not used much now?
Due to sensitivities like burning sensation, redness, SPK
40
Ethylenediamine tetra acetate (EDTA)
Chelating agent, not a true preservative
41
Why is EDTA used in combo with other agents? What is this combo used against especially? MOA?
Synergistic effects of combo; used especially against Pseudomonas; MOA - disrupts cell walls of bacteria to give preservative better efficacy
42
Polyaminopropyl biguanide (PAPB/Dymed)
Used in GP and SCL solns (Conc 30-50x greater in GP solns)
43
More or less effective than Chlorhexidine against Serratia?
PAPB has Greater efficacy
44
PAPB is effective against ?
Acanthamoeba
45
Examples of PAPB?
Boston Advance Conditioning Solution, Boston Simplus, B&L soft lens products
46
Polquaternium-1 (polyquad)
Similar molecular structure to chlorhexidine (So, bactericidal); used in GP and SCL solns
47
Polyquad is found in: (3)
1. Unique pH 2. Opti-Free products for SCLs 3. Most Alcon preserved ATs
48
Benzyl Alcohol
Good disinfection capabilities; negligible binding to GP lens surface
49
What is benzyl alcohol effective with?
Lipid Removal
50
Example of Benzyl alcohol?
Optimum CDS Solution

53
B&L Boston Line Condition Solution
-Disinfects and cushions -Wetting agent that coats surface of lenses to make lenses smooth, wet, and comfortable -Helps reduce friction between lens and eye
54
Boston Original preservative? Viscosity?
Chlorhexidine; very thick viscosity
55

Chlorhexidine (less than Original) and PAPB/Dymed; viscosity is less thick than Original
56
Alcon Clear Care (Hydrogen Peroxide)
FDA approved use with GPS; NON-preserved
57
Rub for Clear Care vs Clear Care Plus?
Required for Clear Care (GPs) but no rub required for Plus (just 5 sec rinse)
58
Clear Care cannot be directly.....?
Cannot be placed into eye directly from bottle or before neutralization (wash hands after rubbing)
59
Menicon Unique pH
Thin in bottle, thickens on eye due to ocular pH (enhance wettability)
60
What preservative in Menicon Unique pH?
Polyquad; Also has H-Guar gelling agent (found in Systane also)
61
Is Menicon Unique pH okay for Hydra-PEG?
Yes
