format of consultation and relevant parts to ask for each
intro- what is their understanding of this apt, how are they in general, how long have they had CKD for and have they had any issues or symptoms, when was their last review and what happened at that apt
general review- symptom screen, PMH, meds (and compliance), allergies, social history
what do we want out of this apt- do we need to make any changes (lifestyle, meds), when will we have next review
safety netting- sick day rules (what meds to stop), come back if they have any issues
what symptoms do you screen for
fatigue and breathlessness
cramps, bone pain
poor appetite and weight loss
nausea and vomiting
explaining management of CKD in patient language
there is no cure for CKD
kidneys can do what they need to do without functioning at 100%
we want to use certain medications and lifestyle modifications to prevent further deterioration of the kidneys where we can
some people get symptoms such as itch, fatigue etc as kidney function gets worse, its important we know about these and investigate and manage them
what medications are routinely used in CKD
ACEis and ARBs can be used to protect the kidneys by reducing the pressure on them (these must be stopped when you aren’t well)
in some people, SGLT2 inhibitors are used to further protect the kidneys
