Chronic kidney disease Flashcards
1
Q
What is the classification of CKD?
A
Based on GFR (normal = 125) CKD 1: 72-90mls/min/1.73m2 BSA CKD 2: 60-89 CKD 3: 30-59 CKD 4: 15-29 CKD 5: <15
2
Q
What are the implications of CKD in the ICU?
A
Altered pharmacokinetics
Fluid and electrolyte disturbances
Cardiovascular - hypertension/ CAD
Haematological - anaemia, plt dysfunction
Neurology - Polynueropathy
Impaired immune function
Dialysis access is a consideration
May be unable to do normal intermittent haemodialysis due to it being inappropriate or unavailable
Conversion to RRT may require alternative access
3
Q
How does CKD alter pharmocokinetics?
A
Altered volume of distribution
Decreased renal clearance
Decreased protein binding
4
Q
What fluid and electrolyte changes occur in CKD?
A
Hyperparathyroidism
Hyperphosphataemia
Acidosis
Risk of fluid overload
