Once identified we will provide those patients with the above criteria, with lifestyle recommendations to help prevent diabetes such as applying a good healthy diet with helpful nutrition information and daily exercise tips.

Project Goal - Determine the amount of patients with a history of hypertension that in the past year :

have been admitted to ER due to the occurrence of high blood pressure i.e. with systolic levels 150 - 200 (or higher than 140/90)
have not had an annual check up with the clinic or we have not collected vitals from them for the last 18 months.
according to their daily BP log have a higher than 140/90 average documented for longer periods of time (i.e. up to 5-7 days at a time)
according to their daily medication log skips their medication 2-3 days or more at a time.
