Chp 1 Flashcards
What’s the conceptual framework for clinical practice?
Is logical structure that organizes clinical practices related to assessment & treatment
Results into a comprehensive plan
Provides guidelines
what are the 4 concepts that contribute to a conceptual framework
Clinical decision-making
Hypothesis oriented clinical practice
Model of disablement
Theory of motor control
What are the steps of Effective Clinical Decision Making (Just think of a the steps to an Eval)
Involves interrelated steps
Leads to the planning of effective treatments
Examination of patient
Evaluation of the data & problem identification
diagnose determination
Prognosis generation & POC (plan of care)
Implementation of POC
Reexamination of outcomes
Purpose of Clinical Decision making
To establish scientifically sound and cost effective plan of care geared to the problems of the individual
Hypothesis Oriented clinical practice is?
Can be defined as a proposal to explain certain facts
Reflect theories about cause and nature of function and dysfunction
Hypothesis Oriented Algorithm for Clinicians (HOAC) 2 steps
Part 1: eight guides to planning intervention
Part 2: outcomes and evaluations
Model of Disablement helps to asses?
Suggests a framework for structuring the effects of disease
Helps to answer “what shall I assess”
ICDH Model vs Nagi model…NO not again :(
ICIDH - Pathology- Impairment-Disability-Handicap
Nagi - Pathology-Impairment-FL-Disability checkout pg box 1-3 Textbook pg 9
Disability is a? and its man categories are?
Inability to engage in age specific roles in a particular environment Results from an impairment 4 main categories of function are Physical Mental Social emotional
Handicap for ICIDH vs Nagi Disability
Social disadvantage resulting from an impairment or functional limitation
Nagi offers
functional limitation as restrictions in performance, physical action, activity, or task
Disability as an inability to engage in specific societal roles
Plan of Care expected Goals? Goals should be
Objecttive, Measurable, Time limited
what are the 4 essential elements to a goal
- Individual - who is performing the action
- Behvaior/ Activity- What is the specific activity that the pt. will perform. Will the pt. play tennis, golf, box, run around naked doing round house kicks with chuck Norris. ALWAYS include the improvement the pt. made for example did he increase his ROM or MMT.
- Condition- Where and when will the pt. perform the task.
- Time- Long Term greater than 3 weeks. Short term less than 3 weeks.
Difference between goals and outcomes
Outcomes= define patient’s expected level at the end of the episode of care Goals= define the interim steps needed to achieve the outcome
3 steps to the intervention
3 components
1. Coordination, communication, documentation (always document what you are going to do and coordinate and communicate with other PT’s
2. Patient/client-related instruction- Make sure the pt. knows what to do even if they are Spanish and just say “yes” to everything they probably don’t understand avoid the lawsuit get a translator.
3. Procedural intervention- Here’s the actual intervention part.
Therapeutic exercises
Schema : FITT, Posture/activity- technique – required elements
Patient Participation in Planning Free choice- MC choice Confirmed choice Forced Choice No Choice ' (Explain Each)
1.5 pg. 15- All this box is saying is ask the patient what they want out of therapy and what is most important to them keep the pt. involved.
1.6 - box 1.6 Free choice- open ended question
MC= Give Pt. a few options to choose from for treatment
Confimed Choice- Therpaist ask questions makes a “suggestion” then lets pt. choose
Forced Choice- The therapist asks a question and Pt. can agree or disagree with what has been picked
NO choice- The PT just does what he or she wants and doesn’t involve the patient what so ever.
