How do you diagnose HF
1) BNP
2) 2D echo to see: EF, pulmonary artery function, diastolic pressure
3) left hear cath (shows if ischemic HF or nah)
If ___ is not elevated then a decreased chance for HF
BNP
Systolic or diastolic HF has decreased EF and edema
Systolic
HF class I,II,III,IV
Drugs to treat, just add on as you go down
I: no symptoms: B blocker, ACE/ARB
II: go about activities of daily living: + loop diuretic
III: SOB with moving + isosorbide dinatrate hydralazine and spironolact
IV: SOB and rest + inotrops like dobutamine and milrinone
When do you use an AICD
In someone with class 1-3 HF and has an EF under 35%
All classes of HF need to follow these
No smoking
Less than 2L water a day
Less than 2g NaCl a day
Test for chf exacerbation
CXR, ECG, BNP, troponins
CHF exacerbation with ECG and troponins showing STEMI treatment
MONA BASH
Morphine, oxygen, nitrates, ASA, b blocker, ACEI, statin, heparin
CHF exacerbation with no ECG or troponin showing MI or STEMI but shows CHF treatment
