Helper (Modified Dependence) Score Description 5 Supervision or Setup (subject = 100%) 4 Minimal Contact Assistance (patient can perform 75% or more of task) 3 Moderate Assistance (patient can perform 50% to 74% of task)
Helper (Complete Dependence) Score Description 2 Maximal Assistance (patient can perform 25% to 49% of tasks) 1 Total assistance (patient can perform less than 25% of the task or requires more than one person to assist) 0 Activity does not occur

LEVEL 1: All (Automatic actions)- automatic motor responses (walking, standing, drinking, eating), total assist, sensory stimulation
LEVEL 2: Post (Postural)- think of movement associated with comfort and GROSS movements (bending, stretching, pacing), max assist, able to notice large objects in environment, attention span minutes
LEVEL 3: Men (Manual)- think of MANipulation with hands, Can imitate running stitch, mod assist, can perform simple tasks with repetition, very distracted, can perform familiar ADLs; attention span 30 minutes
LEVEL 4: Go (Goal directed)- imitates whip stitch; perform simple tasks/crafts through completion with heavy reliance on VISUAL CUES; can carry out established routines (cannot cope with new or unexpected); min assist, at level 4.6 can live alone; attention span hours
LEVEL 5: Explore (Exploratory)- simple cordovan stitch, overt trial and error problem solving (needs to perform action and learns trial and error by doing), independent and NEW LEARNING occurs, poor organization, planning, and socialization, activities: concrete tasks
LEVEL 6: Places (Planned)- absence of disability, independent
