Circle area in red on appropriate tooth surface Label “abras”

Add a notation on the chart regarding the supernumerary tooth, in the area it is located
Unerupted (Impacted) Tooth

Abbreviation: IMP
Circle all views in blue for impacted teeth that are NOT indicated for extraction
Extraction indicated for impacted tooth
Circle all views in red and place red “x’ over crown and root views
Caries
For primary, root, or recurrent caries, outline approximate size of lesion and fill in with red
Endo
Draw a blue line through root to the extent that gutta percha is seen on radiograph
