adds detail pertinent to the encounter with the provider of care and is the seventh character in an ICD-10_CM code
First-listed diagnosis
used by health care providers in a non-hospital setting to identify the main or chief reason why a patient is being treated or receiving care
ICD-10-CM
International classification of diseases, tenth revision, clinical modification. Provides diagnostic codes for diseases, injuries, and other conditions
Manifestation
the outward symptoms and signs of a disease or condition
NEC
Not elsewhere classified. The category number for a code including the description NEC can only be used when the code lacks information necessary to code the term to a more specific category
NOC
not otherwise specified; the diagnostic statement does not provide more specificity or definition
parentheses
parentheses are used in both the index and tabular list to enclose supplementary words which may be present or absent in the statement of a disease or procedure without affecting the code number to which it is assigned
placeholders
the letter x is used to fill in an empty space between the end of a code and the seventh character extension
principle diagnosis
that condition established after study to be chiefly responsible for occasioning the admission of the patient to the hospital for care

directs the coder to additional main terms in the alphabetic index

refers the coder to a different location in the alphabetic index
sequela
late effects or complications
subcategory
adds additional detail or specificity to the category code
tabular list
contains all codes, in alphanumeric order, and code descriptions, subdivided by chapters
with/ without a code description
indicates whether there is an additional problem associated with the codes
