Where does the suprascapular artery branch from?
Branch of the thyrocervical trunk which branches from subclavian. Runs over transverse scapular ligament
Where does the dorsal scapular artery branch from?
Branch right off of subclavian, distal to thyrocervical trunk
Between which scalene muscles does the subclavian pass through?
Anterior and middle
What are the branches of the 1st part of axillary artery?
Superior thoracic a. Supplies serratus ant and pe muscles
Branches of 2nd part of axillary artery
Thoracoacromial (with its 4 branches, clavicular, acromial, deltoid, pectoral) and Lateral thoracic, to serratus anterior
What branch of thoracoacromial artery is at risk during subacromial decompression?
Acromial branch
What branch of thoracoacromial artery is at risk during clavicular fx or surgery?
clavicular branch
What branch of thoracoacromial artery is at risk during deltopectoral approach?
deltoid branch
What are the 3 branches off the 3rd part of axillary artery?
Subscapular (with its branches circumflex scap and thoracodorsal), posterior circumflex humoral and ant circumflex humoral arteries
What artery is the primary supply of the humeral head?
The arcuate artery of the anterior circumflex humoral artery
What breaks up the axillary artery into its 3 parts?
Borders of pec minor. 1st part prix, 2nd part behind, 3rd part distal
In a rotator cuff tear from a chronic process, where is the tear located?
Usually on the superior side of the cuff, the bursal side
In acute rotator cuff tear, where is tear usually located?
Usually on articular side, maybe in athletes or after dislocation
What might be something in hx of shoulder pain to clue in rotator cuff tear?
Pain at night
what radiographic appearance on MRI may indicate cuff tear?
Contrast shows communication b/w joint and subacromial space
What is a TUBS lesion?
Acronym for shoulder instability. Trauma (usually dislocation) Unilateral and unidirectional, Bankart lesion (tear of ant. inf labrum from dislocation) Surgery (if not 90% recurrence rate)
What is an AMBRI lesion
Acronym for shoulder instability. Atraumatic Mulitdirectional Bilateral (one side usually worse) Rehab (responds to rehab) Inferior capsular shift may help
What caues medial scapular winging?
serratus ant weakness, usually due to long thoracic n palsy
What causes lateral scapular winging?
Trapezius weakness, 2ary to spinal accessory nerve palsy
What is a SLAP lesion and what test to diagnose?
Superior Labrum tear from Ant to Post. Diagnose with + Obriens test and crank test
Thoracic outlet syndrom caused by compression where?
1st rib and clavicle, between scalene muscles. May be associated with cervical ribs
What is Sprengel’s deformity?
Small hypoplastic undescended scapula. Omovertebral bone connects Cspine to scapula
What is a major danger of anterior deltopectoral approach in surg?
Musculocutaneous N damage from vigorous retraction of conjoined tendon
