5E model
A model of learning that promotes active rather than passive patient education. The model includes the following five stages: Engage, Explore, Explain, Elaborate, and Evaluate.
action phase of change
In the transtheoretical model of change, at this stage the patient is actively engaged in changing behavior less than 6 months.
affective patient education
Patient outcomes that focuses on changing a patient’s attitudes or values.
assessment
Collecting the data upon which a diagnosis can be based. It is the process of identifying the signs and symptoms of disease and recording the associated data.

An international network that provides information to help healthcare providers, policy makers, patients, their advocates, and care providers make well-informed decisions about health care.
Cochrane Library
An online collection of databases that brings together in one place rigorous and up-to-date research on the effectiveness of healthcare treatments and interventions, as well as methodology and diagnostic tests.
cognitive objectives
Outcomes that focus on changing a patient’s knowledge.
community health initiative
A program or project aimed at improving health at a community or subcommunity level.
contemplation phase of change
In the transtheoretical model of change, at this stage the patient is considering action to change behavior in the next 6 months.
culturally competent care
According to the Office of Minority Health, it is a set of congruent behaviors, attitudes, and policies that come together in a system, agency, or among professionals.
e-health
The use of technology to communicate information to improve health and health services.
evaluation phase of care
A stage in the patient care process, following the implementation phase, in which the goal is to determine the patient’s progress with the intervention.
health belief model
An approach to behavior change designed to influence individuals to take action to engage in healthy behaviors.
health communication
The study and use of communication strategies to inform and influence individual and community decisions that affect health.
health literacy
The degree to which individuals have the capacity to obtain, process, and understand basic health information and services needed to make appropriate health decisions.

An online database sponsored by the National Health Information Center that contains health topics and other databases to locate physicians and other providers and healthcare centers.
maintenance phase of change
In the transtheoretical model of change, at this stage the patient has sustained the behavior change for more than 6 months.

A consumer health information website developed and maintained by the National Library of Medicine and the National Institutes of Health. The information provided is free.
National Health Education Standards (NHES)
National standards for health education that articulate the framework for promoting health at the personal, family, and community levels for students in pre-kindergarten, elementary school, middle school, and high school.
numeracy
Application of numbers and measurement into daily tasks such as medication dosing.
patient-centered health care
A model for health care in which the needs, preferences, goals, capabilities, and resources of the patient or population are at the center of healthcare decisions.
planning phase of patient care
During this phase of patient care, the patient shares in the decision-making process when the clinician presents his or her findings during the case presentation and gains informed consent to proceed with care.
point-of-care products (or resources)
Health information search tools that contain synthesized evidence useful to the practitioner while treating the patient.
precontemplation phase of change
In the transtheoretical model of change, at this stage the patient is not willing or ready to change behavior within the next 6 months.
preparation phase of change
In the transtheoretical model of change, at this stage the patient is preparing for the change within the next month and is partially engaged in changing.
psychomotor objectives
Outcomes that focus on changing a patient’s motor skills.
