Primary vs Secondary vs Tertiary Care
Primary = responsible for majority of healthcare needs Secondary = provided by clinicians on referral basis Tertiary = provided by specialists
Examples of primary, secondary, and tertiary care
Primary = direct access; seeing patient as first point of contact Secondary = private practice led therapist treating child referred from pediatrician Tertiary = PT treating patient in specialty burn center
Patient Client Mangement Model
Examination –> Evaluation –> Diagnosis –> Prognosis –> Intervention –> Outcomes
Examination
First component of Patient Client Management model; consists of history, systems review, and tests and measures
Evaluation
PTs clinical judgment based from examination
PT continually evaluating to determine if an adjustments are necessary throughout the episode of care
Diagnosis
Evaluation used to establish diagnosis
PT diagnosis focus on impact on function using diagnostic label
Prognosis
Prediction of level of improvement patient will make and time necessary to reach that level
Intervention
Plan of Care = designed plan consisting of goals, outcomes, intervention, frequency, and duration of treatment
Goals = established w/ patient to provide ways to measure progress and determine appropriate time for discharge
Intervention = PT procedures are conducted w/ patient or client to achieve desired outcomes
Outcomes
Results from interventions and entire POC; data collected and analyzed to enhance quality assessment
