Comprehensive Health History
Establishes complete database.
Problem-based/Focused Health Assessment
Consists of data limited to the scope of problem.
Episodic/Follow-up Assessment
Consists of specific problems for which patient is already receiving treatment.
Health History
subjective data, involves interview of the PT and questionnaires; a database used to create a plan, prevent disease, resolve problems, and minimize limitations
Open Ended Questions
Encourages a free-flowing, open response; these questions often focus on PT’s health. e.g. How are you doing?
Close Ended Questions
Yield more precise data, gives options for response. e.g. Do you become short of breath?
Directive Questions
Lead PT to focus on one set of thoughts, often used in reviewing systems and evaluating functional status. e.g. Describe the drainage you have been having from your nose.
