Chapter 13: Disorders of the Spleen Flashcards
What are the 4 functions that impact disease states in the spleen?
- phagocytosis of blood cells and particulate matter
- Antibody production
- hematopoiesis
- sequestration of formed blood elements
What is the one major clinical manifestation of splenic insufficiency?
An increased susceptibility to sepsis caused by encapsuated bacteria such as pneumococci, meningococci, and H influenza
Splenomegaly
dragging sensation in the left upper quadrant
Nonspecific acute splenitis
spleen is enlarged and soft
-major feature is acute congestion of the red pulp
Congestive splenomegaly
- chronic venous outflow obstruction
- cirrhosis of the liver can cause it
- pipe-stem hepatic fibrosis of schostosomiasis are cause a really severe congestive splenomegale
- organ is firm
- elevated portal venous pressure stimulates deposition of collagen in the basement membrane of the sinusoids
Splenic infarcts
- occlusion of the major splenic artery or any of its branches
- wedge-shaped and subcapsular
Neoplasms
-rare except in myeloid and lymphoid tumors
Congenital abnormalities
Complete absence of spleen is rare…. hypoplasia is more common
-Accessory spleens are common… cause hematologic disorders
Rupture
- blunt trauma
- infectious mononucleosis, malaria, typhpoid fever, and lymphoid neoplasms
