1. Create a small gastric pouch from part of the stomach, disconnected from the duodenum.
2. Small loop of bowel is surgically joined to the gastric pouch
3. Distal segment of small bowel is brought up and the upper part reconnected lower to avoid reflux with unidirectional peristaltic flow
