Chapter 124: Urologic Emergencies - Incomplete Flashcards
At when age do foreskin adhesions spontaneously lyse in 90% of children
2-4 years
Definition of phimosis
Tightness of distal foreskin precludes its being withdrawn to expose the glans
What condition is commonly confused with phimosis
normal penile adhesions
Definition of paraphimosis
In uncircumcised male infant if the foreskin is retracted behind the glans and left in that position, venous congestion and edema of the foreskin results - making it difficult to reduce
Non-surgical management options for paraphimosis
Ice, and steady local manual compression
Topical anesthestic or penile block - ( then turn the sock in side out)
Surgical management of paraphimosis
Surgical division of the foreskin - followed later on with circumcision
Balanoposthitis
Infection of the foreskin the spreads to the glans - a form of cellultis - often due to a break in penile skin and ammoniacal dermatitis
Causes of balanoposthitis
Local trauma or poor penile hygiene
Treatment of balanoposthitis
Warm soaks, ampicillin
Patient may feel more comfortable voiding in a tub of warm water
Circumcision if recurrent events
Priapism causes
Sickle cell (sludging and stasis in erectile tissue of corporal bodies - hypoxia - acidosis - sickling Leukemic infiltration
Complication of priapism
impotence ( but is rare when related to sickle cell disease)
Treatment of priapism associated with sickle cell disease
Hydration and irrigation of corporal bodies along with vasoactive substances
True or false meatal stenosis is almost exclusively a problem of the circumcised male
