Chapter 11 Flashcards
No break in sterile technique, no wound drain, no entry into 5 systems. What type of wound class is this?
Class I : Clean Wound
Wound drained, minor break in sterile technique, controlled entry into 5 systems. What type of wound class is this?
Class II : Clean Contaminated
Open traumatic wound, major break in steril technique, entry into 5 systems. What type of wound class is this?
Class III : Contaminated
Open traumatic wound, microbial contamination, perforated viscus. What type of wound class is this?
Class IV : Dirty / Infected
Closed Wound
skin intact but underlying tissue suffers damage
Open Wound
Integrity of skin is destroyed (4 types)
Simple Wound (Open)
integrity of skin destroyed. no loss or destruction of tissue. no foreign body in wound.
Complicated Wound (Open)
tissue lost or destroyed or a foreign body remains in wound
Clean Wound (Open)
would edges can be approximated. expected to heal by first intention.
Contaminated Wound (Open)
dirty object damages integrity of skin. can become infected within short period of time. debridement or excision of infected and/or necrosed tissue may be necessary.
Wound Classification by Mechanism of Injury
Abrasion: scrape Contusion: bruise Laceration: cut or tear Puncture: penetration Thermal: heat or cold (can be chemical)
Chronic Wounds
wounds that persist for an extended period of time
First Intention: Primary Union
- incision opened under ideal conditions
- healing occurs from side to side
- dead space eliminated
- wound heals with no separation and minimal scarring
- 3 Phases: Lag, Proliferation, Maturation
Phase 1: Lag (inflammatory response)
- 3-5 days
- inflammation, heat, redness, pain, swelling, loss of function.
- bleeding controlled by platelet aggregation, vessel dilation to deliver blood, and formation of epithelial cells for repair.
Phase 2 : Proliferation
- days 3 to 20 post op
- fibroblasts multiply & bridge wound edges
