The Five Core Elements of MTM

A

MTM consists of five core elements: medication therapy review, intervention and referral, personal medication record, medication-related action plan, and documentation and follow-up; each of which can be modified to fit the needs of the individual patient.

Medication Therapy Review (MTR)
This type of medication review can also be called a comprehensive medication review (CMR).

A

A medication therapy review is a one-on-one consultation between the patient and pharmacy professional, in most cases a pharmacist. However, a pharmacy technician or intern may collect and document the patient’s medication list, over the counter (OTC) medications, medical history, allergies, adverse drug reactions (ADR), and any other necessary information to gain an overall better understanding of the patient’s complete medical history.
