Acute periodontitis:
Suppurative form
1-Periapical abscess , periapical Osteitis
2-More pronounced symptoms and malaise (virulence, reduced host defenses, trauma)
3-Vitality negative+pus
Chronic periodontitis:
Callous type
Fibrous tissue not localized (healing)
Chronic periodontitis:
Granuloma type
1-Hyperplastic defensive inflammatory periapical lesion
2-Slight/Weak but continuous/repeated stimulus
3-No symptoms
4-Rx: appearance of periapical or lateral radiolucency
5-Destruction of cement, plate, and bone
6-Darker area: Radiolucency
7-Malassez-Epithelial Granuloma-Cysts
Cellulitis = Phlegmon
1- Inflammation of the soft tissue of the mouth, face, and neck

3-On palpation: Hard and doughy edema
4-Not localized infection, there are no anatomic limits
5-There is no tissues destruction, there’s no suppuration

Abscess
1-When the host’s defense is failing or increased virulence of the micro-organism
2-Sudden appearance of localized suppuration
3-Limited/with boundaries
4-Sudden appearance in any cervicofacial area
Abscess: Clinical manifestations with much emphasis
1-Red swelling
2-Less pain (The organism is fighting the infection)
3-Fluctuation
4-Fingerprint sign
5-Functional and Bad condition
FACTORS THAT INFLUENCE THE SPREADING OF INFECTION
- Suppuration in the periodontal space (viewed )
- Direct dissemination from lymphatic, venous or parulic route (sub-periostic)
- Systemic diseases decreasing defenses
GENERAL FACTORS THAT INFLUENCE THE SPREADING OF INFECTION
- Balance between resistance and agressiveness
- Host resistance: Humoral and cellular defense- Bacterial action
- Aerobic
MICROORGANISMS INVOLVED IN MIXED BACTERIAL INFECTIONS OF THE ORAL CAVITY
The odontogenic infection is ,biologically dinamic , polymicrobialand can be mixed with predominatinganaerobic flora
LOCAL FACTORS THAT INFLUENCE THE SPREADING OF THE INFECTION
- Socket situation and cortical plate thickness
- Root length and attached mucose
- Muscle insertions-
- Movements of the region
- Anatomic spaces and aponeurotic layers (fascia)
- Anatomic regions
If the apex is higher than the insertion
