It’s a pressure sensing device.

What is the difference between pressure altitude and real/true altitude?
A
True altitude is your direct height above mean sea level regardless how far from the ground you are.
Pressure altitude is the altitude as determined by the pressure of the air where the aircraft is.

What happen to the transition altitude once it is given to you?
A
It hardly ever changes; but transition layer does.
