What are the different types of pharmacist licenses?
Active: was renewed by dec 31st
Inactive: Pharmacist requested to be inactive prior to dec 31st
Expired: Was not renewed prior to dec 31st
What can military folks receive?
They must make a decision for their license within 30 days and they can receive a special work pass (120 days) if they ask.
Who qualifies for a special work pass?
They must have all other pharmacist qualifications, be active duty or dependent (<24 in college, < 21 not in college, financially dependent, or spouse)
Person may be civil service
Must hold license in another jurisdiction for at least 1 year
Must be stationed in LA
How many hours of CE are required for pharmacists every year?
15 hours of CPE per year (3 are live)
OR
20 hours of CPE per year (if less than 3 hours live)
What must the pharmacist document on for their CEs?
CPE monitor
When must you notify the board if you move or change employment?
10 days
What is an impaired pharmacist?
Alcoholism, substance abuse or addiction, mental illness, or physical illness

90 days and must be written down with the words “emergency prescription”
Can outside pharmacists work in La during a state of emergency?
Yes they must obtain a special work permit from the board and must do legit relief work. They must notify the board of time in state and approximate location and must have some sort of proper ID
Can a pharmacist administer medications?
Yes if the physician writes an “authority to administer” prescription
The authority to administer document must have?
Practitioner’s name, address, and phone number
Patient’s name, address, gender, and DOB
Med, dose, route
Pharmacist name
Date of original administration
What are the requirements of the pharmacist to administer a vaccine?
Be licensed with the board.
Complete a APhA course (20 hours)
AHA BCLS (BLS)
Provide documents of completion to the board, display certificate, and must have 1 hour in field each year/
Renewal is tied to BLS certification
When should an immunization be documented in LINKS?
within 72 hours
What happens if a pharmacist fails to report to links?
They will be punished per vaccine administered
When should immunizations be notified to PCP of patient?
within 24 hours.

What is collaborative drug therapy management?
This allows pharmacist to coordinate with a physician to care for the physician’s patients.
What are the examples of CDTM?
Monitoring/modifying specific drug therapy
Collecting/reviewing patient history
Obtaining/reviewing vital signs
Ordering/evaluating/applying results of lab work
Providing disease or condition specific patient education and counseling
How does on register to provide CDTM?
Must have a pharmacist license and must be actively practicing in the state.
When does a CDTM cease to exist?
When a pharmacist or physician dies, loses license, action against either license, or notify they don’t want to do it any longer
Who may NOT be a CDTM patient?
Any patient without a patient specific physician plan
Under 18 yo (except smoking and immunizations)

Have a drug substituted for another without the consent of physician
What do you have to tell the patient about the CDTM?
Patient must understand they are being treated by a pharmacist
They have a right to refuse enrollment
