Ch 34 IUD Trivia Flashcards
If an IUD cannot be found, what other scan can be performed to reveal the location of the migrated IUD.
Abdominal radiograph or CAT scan
If patient is having pain, where else should we look and why?
The adnexa to rule out an ectopic pregnancy
This can happen if the IUD is placed incorrectly.
When in the _______ phase, why is it difficult to see the IUD?
Secretory because the endometrium is thickened.
A patient comes in the office and says that she cannot feel her IUD. Sonogram shows that the IUD has perforated the uterus and is now in the pelvis. The doctor ordered a radiograph.
- What is this pathology called?
- Why would they order a radiograph?
Lost IUD with a complete perforation
A radiograph will show whether or not the IUD is still within the patient.
It is important to show the relationship between the IUD and the endometrial cavity.
If the IUD is in contact w/ endometrial cavity, can the IUD be withdrawn?
yes
It is important to show the relationship between the IUD and the endometrial cavity.
If the IUD is entirely in the myometrium, can the IUD be withdrawn?
No, the uterus would have to be surgically removed.
IUD’s can be associated w/ an increased incidence of ______.
Pelvic Inflammatory Disease
A patient comes in and presents w/ pain and bleeding. We check the IUD, and it is correctly positioned.
What should we then check for?
Check the adnexa and cul-de-sac for evidence of Pelvic Inflammatory Disease. Occasionally an abscess around a perforated infected IUD allows an extrauterine IUD to be seen.
Perforation of the uterus by an IUD can be either complete or incomplete.
What will an incomplete perforation look like?
A portion of the IUD may be seen within the uterine wall.
Pregnancy’s with an IUD are almost unknown with modern IUD’s because?
They contain progesterone-like substances
When an IUD w/ a coexisting pregnancy is discovered, what should we do?
We should determine the relationship b/w the device and gestational sac. (For example, superior or inferior)
If an IUD is left in place during a pregnancy, a severe infection may occur. No question just know.
Just know!
In the later stages of pregnancies, the location of IUD can be difficult because
the large volume of uterus is occupied by the fetus.
In the long axis view, the ______ has 2-5 echogenic components Transversely it is seen as a single line.
Lippies Loop
Largely associated w/ infection. the smallest of the IUD’s; On long and trans, it appears as 2 echogenic foci.
