ch 29 - periodontal surgical concepts for the dental hygienist Flashcards
resective
procedures that simply cut away and remove some of the periodontal tissues
osseous defect
a deformity in the tooth-supporting alveolar bone usually resulting from periodontitis
relative contraindication
a condition that may make periodontal surgery inadvisable
repair
healing of a wound by formation of tissues that do not fully restore the original architecture or original function of the body part
reattachment
means to “attach again,” the healing of periodontal wound by the reunion of the connective tissue and roots where these two tissues have been separated by surgical incision or physical injury, but not by disease
new attachment
a term used to describe the union of a pathologically exposed root with connective tissue or epithelium
regeneration
the biologic process by which the architecture and function of lost tissue is completely restored
primary intention
occurs when the wound margins or edges are closely adapted to each other
ex - small wound in a finger that required stitches
secondary intention
occurs when the margins or edges of the wound are not closely adapted, the two wound edges are not in close contact with each other
ex - tooth extraction socket
tertiary intention
occurs when the wound is first cleaned by the surgeon and left open deliberately to ensure no infection is apparent before it is closed at a later date
ex - wound from a dog bite
periodontal flap
a surgical procedure in which incisions are made in the gingiva or mucosa to allow for separation of the surface tissues from the underlying tooth roots and alveolar bone
flap elevation
separating the surface tissues from the underlying tooth root and alveolar bone
full-thickness flap
provides complete access to underlying bone when bone replacement grafting or periodontal regeneration procedures are anticipated
blunt dissection
elevation of a full-thickness flap
partial-thickness flap
elevation of only the epithelium and a thin layer of the underlying connective tissue
sharp dissection
incising of the underlying connective tissue in such a manner as to separate the epithelial surface plus a small portion of the connective tissue from the periosteum
nondisplaced flap
a flap that is sutured with the margin of the flap at its original position in relationship to the CEJ on the tooth
displaced flap
a flap that is sutured with the margin of the flap placed at a position other than its original position in relationship to the CEJ of the tooth
horizontal incisions
directed along the gingival margins in a mesiodistal direction
crevicular incision
surgical scalpel blade is carefully placed into the gingival sulcus and directed apically to bone
