Alphabetic Index
one of two ways diagnoses are listed in the ICD manual. They will appear in alphabetical order with their diagnosis codes.
Category
the first three digits of the diagnosis code in ICD-10.
Chapters
the breakdown of diagnosis codes by body system or disease.
Chief Complaint
the patient’s main concern/issue of pain or ailment
Combination Code
an ICD code in which two diagnoses are included in one code.
Convention
a list of punctuations, abbreviations, typefaces, symbols, and instructional notes appearing in the beginning of the ICD-10. It provides the guidelines for using the code set.
Cross-reference
The notation within the ICD-10 of the word “see” after a main term in the index. The reference of “see” means that the main term first checked is not correct and another category has to be used.
Diagnosis
(Dx) the primary condition for which a patient is receiving care.
Etiology
the science and study of the causes of disease and their mode of operation.
International Classification of Disease
code set that is based on a system maintained by the WHO of the United Nations. ICD-10 codes allow for more specificity and accuracy when coding.
Laterality
- the side of the body affected by the diagnosis in ICD-10.
Morbidity
frequency of the appearance of complications following a surgical procedure or other treatment; a disease state.
Mortality
A fatal outcome
Primary Diagnosis
the diagnosis given as the primary reason for the patient seeking care.
Principal DIagnosis
the diagnosis that is found, after testing and study, to be the main reason for the patients need for healthcare services.
