What are Reportable Criteria (Diving on duty) MISHP
- All recompression treatments
- Any incidence of Type I or II DCS
- All Cases of POIS
- Any case of loss of consciousness
- CNS or pulmonary O2 Toxicity
Who do we report MISHAPS to?
Reported to NAVSAFECEN
What are HAZREPS (Hazards and near mishaps) (Diving on duty)
- Unplanned shift to secondary air
- Aborted dive due to unexpected issue/event
- Trapped/Fouled diver where standby or buddy
diver was required - Lost diver where stand-by diver was required
- Exceeding prescribed limits
o Maximum depth
o Bottom Time
o Omitted decompression
o O2 exposures above allowed pulmonary 02
limits - Any abnormal conditions discovered
o C02 canister installed or filled improperly
o C02 canister not installed
o Exhaust valves installed improperly
o Dive system aligned improperly - Any external
o Ships tags cleared without proper
authorization before, during, or after divers
enter the water
o Unauthorized cranes operated overhead of
diver
o Small boat/craft operation conducted over or
in the vicinity of diver
o Unauthorized discharge/SONAR while diver
are in the water
How are HAZREPS reported?
No SIREP, but reported IAW OPNAVINST 5102.1
Where did the shipping of the dive equipment Mishap come from?

NAVSEA 00C3 will make the determination as to if the equipment is involved in the Mishap will need to be shipped. They have outlined everything in the checklist available on the Secure SUPSALV.
Timeframes for Mishap For a Class “A” or “B”
The initial notification to the CG-DCO-NCC is 5 minutes, the Preliminary Database Entry is 12 hours.
Timeframes for Mishap For Class “C” “D” “E”
The Preliminary Data Entry is 24 hours for operational and High Potential (for loss) HIPO
What are CG reportable diving mishaps? Dive Specific
Dive Specific
- Hyperbaric injuries requiring recompression
therapy
- Hyperbaric injuries requiring no recompression
therapy but render the member NPQ
- Mechanical or other injuries requiring medical
treatment that render member NPQ
- Illness caused by pollutants in the water
