What are triple swabs?
High vaginal swab, endocervical swab, endocervical chlamydia swab
What pathogens are found on the high vaginal swab?
1) Bacterial vaginosis
2) Trichomonas vaginalis
3) Candidiasis
4) Group B strep
5) Syphilis (treponema palladium)
What pathogen are found on endocervical charcoal media swab and how is the swab done?
Neisseria gonorrhoea
Place swab in endocervical canal and done a full 360 degree sweep.
What pathogen can be found in the 3rd swab in the triple swab? How is the swab done?
Chlamydia
Scrub endocervial region in and out for 10 -30 seconds.
What is the treatment for bacterial vaginosis?
Metronidazole ( also to treat PID, endocarditis)
What is the treatment for trichomonas vaginalis infection?
Metronidazole
What are the symptoms of trichomonas vaginalis infection?
offensive grey green discharge,
superficial dyspareunia, punctuate erythematous (strawberry) appearance.
What are the symptoms of candidiasis infection?
- Cottage cheese white discharge
- Vulval irritation, inflammation and itching
- Superficial dyspareunia
- dysuria
What are the risk factors of candidiasis infection?
Pregnancy
Diabetes
Use of antibiotics
What are the signs of candidiasis infection?
Red and inflamed vagina and vulva
What are the treatment of candidiasis?
Topical imidazoles - e.g.: Canesten (Clotrimazole)
Oral fluconazole
What are the symptoms of chlamydial infection?
Often asymptomatic
- Vaginal discharge
- Dysuria
- Pyuria
- Vague suprapubic pain
- fever
- IMB, postcoital bleeding
- Deep dyspareunia
What syndrome can chlamydial infection cause and what are its characteristics?
Reiter’s syndrome
- Urethritis
- Conjunctivitis
- Arthritis
What are the clinical signs of chlamydial infection in females?
Inflammed cervix
Follicular, cobblestone appearance in cervix
Cervical bleeding
Abdominal tenderness
Bimanual palpation - Pelvic adnexal tenderness
Cervical excitation
What are the investigations for chlamydial infection?
Nucleic acid amplification tests (NAATs) e.g.: PCR
