what is CBT for AN?

A

enhanced cognitive behaviour therapy is used in the treatment of AN as well as other eating disorders as it is aimed specifically at tackling the thoughts and behaviours associated with disordered eating behaviour.

what is stage 1 of CBT?

A

this stage takes around 4 weeks with 2 sessions per week to encourage rapid change in client’s behaviour. there is:
- weekly weighing with the therapist
- regular eating
clients are educated about the disorder and they will also learn about the treatment programme so they know what to expect
- it is important that the client is positive about the treatment and motivated to progress

what is stage 2 of CBT?

A

this is brief consisting of 2 appoitments, 1 week apart
- the therapist and client will meet to discuss the progress being made in stage 1 and taking stock of how the patient is

what is stage 3 of CBT?

A

this is the main stage of the treatment phase where clients will have 8 appointments, once a week to tackle the factors involved in the maintainance of the eating disorder
- this will involve dealing with the client’s body image, their dietary rules and any event-related changes in eating
- dealing with body image will look at certain behaviours likely to lead to body dissatisfaction such as body checking
- dietary rules are explored to consider the impact that rigid and restrictive rules are having on the client’s quality of life

what is stage 4 of CBT?

A

clients are encouraged to look to the future and consider factors that need to be managed to prevent relapse
- they will usually have 3 appointments about 2 weeks apart
- the therapist and patient will draw up an agreed plan that is personalised for their specific circumstances

what is an advantage of CBT-E?

A

the mechanisms in enhanced CBT are comprehensive enough to be used to treat the changing patterns of disordered eating that are common in patients with eating disorders over time

what is an issue related to the use of CBT-E?

A

the client has to be motivated to change so this is a form of therapy that will not be effective for everyone.
- the most commonly used form of enhanced CBT is only effectiv for patients with AN whose eating disorder is not maintained by clinical perfectionism, low self esteem and interpersonal problems

what is the competing argument of the issue related to the use of CBT-E?

A

the initial stage of treatment does allow for the client to spend time learning about their disorder before they enter the full treatment phase so it will be important that the therapist recognises signs that the patient is not yet ready

they considered the effectiveness of CBT in treating patients with the full range of eating disorders in an outpatient clinic in australia
- they found that two thirds of patients in the treatment group showed significant improvement in the symptoms both in relation to eating behaviour and general psychopathology
