What organism causes Cat Scratch Disease
Bartonella Henselae
What type of organism is Bartonella Henselae
Intracellular, gram negative bacilli
Risk factors for Bartonella
History of kitten/cat contact, transmitted by fleas
Incubation period Bartonella
1-7 days, scratch to papule
Typical CSD presentation
Gradual, regional painful lymph node enlargement (arm or neck)
Clinical course for CSD
Self resolve in 4- 6 weeks, malaise/fever/fatigue in 30% of patients, 25% suppurate, 14% disseminated illness
Parinaud Oculoglandular Syndrome
Unilateral granulomatous conjunctivitis, enlarged regional lymph node
Diagnosis of CSD
Serology (convalescent), culture less useful as intracellular organism, histology
Treatment
Self limiting, resolve in 1-2 months. Treat disseminated/immunocompromised patients with Azithromycin + Rifampicin or Cotrim/Clarithro/Cipro
