Name 3 differences between proposal writing and case report writing.
- Proposal contains the writer’s personal opinion and stand while case report contains facts about the client
- A proposal is written for senior management and does not involve clients while case report involves clients
- A proposal is written to persuade management to accept your personal opinions while case report to make recommendations for clients
Name and explain INCIDENT REPORT, when are they written and for whom are they written for.
INCIDENT REPORT
- Normally written when there is an incident, e.g. unforeseen incident during an outing, fight in a residential home
- Incident report is normally written for the management and sometimes for local authorities if external investigations are required
- Has information such as Name of agency, Date/time/location/nature of incident, Key persons involved, Description of incident, Interim action, Follow-up action, Final action
Name and explain CASE REPORT, when are they written and for whom are they written for.
CASE REPORT
- Written in the context of case management
- Case report written for management, a committee, or the court to assist their review of the client
- Written progressively - At the start of the case to document the client’s background, needs and recommended plans, During the case - to review and document client’s progress and for further recommended plans, At the end of the case - to summarise the client’s progress and achievements
- Has the following information: Client’s data, Case background, Family background and relationship with client, Client’s education/employment, Current development of client, Future actions/recommendations
What is the D principle of case report writing?
DIRECT:
Using fewer words to convey a message or idea across to the audience without changing its meaning, while maintaining the grammatical functions. This is known as reducing redundancy.
What is the O principle of case report writing?
OBJECTIVE:
Facts in reports to be presented objectively. Facts are to be reported as they are, and if necessary, make unbiased recommendations based on facts. Leave out own opinions, values and beliefs when reporting facts.
What is the P principle of case report writing?
PASSIVE:
Using the passive voice and thus removing one’s “voice” and “biasness” from the writing.
