Evaluation Report ( ER)
Multidisciplinary report containing at least two student evaluations completed by the school district; if a student is found to qualify for special education, the educational team has 30 calendar days to complete an IEP
Individual Education Program ( IEP)
Legally binding documents developed annually for students who require specially designed instruction; they identify the special education and related services the student will receive for one calendar year.
Notice Of Recommended Educational Placement ( NOREP)
Document that classifies the type of classroom and programming that a child will receive for the duration of IEP (Individualized Educational Program).
Permission To Evaluate ( PTE)
Documents that parents/guardians are required to sign before the school system may evaluate a student for special dedication services.
Section 504 of the Rehabilitation Act of 1973
Federal legislation that makes discrimination against individuals with disabilities illegal;
Section 504 provides that students who do not require specially designed instruction that may receive accommodations and services including physical therapy if they are considered to have disability that impacts major life function
General Physical therapy plan of care/goals
coordinate transition to kindergarten with related services needed to support the child in her educational program; provide efficient means to access the school environment including classroom, general school building, cafeteria, and playground;
creating meaningful adaptation to promote participation in school activities ( eg:
Physical Therapy Intervention
Coordination and communication with
all members of the educational team; education to school staff including
strategies to promote increased student self-efficacy and safe strategies for assistance with transfers and mobility; procedural interventions for specific goals (e.g., mobility, balance, and strength related to participation
in the school environment and school activities); development of an
emergency evacuation plan; varied positioning during school day to meet
the child’s needs; accommodation of school activities (e.g., physical
education class, extracurricular activities) and environments (e.g., student areas of the classroom, playground) to enhance participation
