what are some eligibly criteria for care home assessment?
A
*unable to do
Managing and maintaining nutrition
Maintain personal hygiene
Managing toilet needs
Dressing appropriately
Moving around the home safely
keeping home safe and clean etc

what are the 2 funding streams available for care home placements?
A
- Social care funding: means tested based on your owned things
- Continuing Healthcare (CHC) - NHS funding

what is CHC funding?
A
Package of care provided by the NHS to some people with long-term, complex health needs
Need to be referred by the council
Individual must have a ‘primary health need’
Funding is not means tested & can be used to
fund a full range of healthcare needs, including care home placements

what is a residential care home?
A
- Provide 24 hour personal care and support for those that need help with ADLs
- Residents may have medical conditions but these do not require frequent medical treatment
- Staff are trained to provide basic care such as administering medications but not provide medical care

what is a nursing home?
A
- Provide 24 hour care for those that need help with ALDs but also have medical conditions that require regular treatment from registered nurses
- Staff are medically trained and can identify symptoms and changes to a resident’s condition
