Transposition of the great arteries
*it’s a conotruncal(outflow) anomaly; it is one of the causes of cyanosis of the newborns
*the transposition creates two parallel circuits between the systemic and pulmonary circulations
*TGA is associated with maternal diabetes
*Dx:
-prenatal US
-cyanosis and metabolic acidosis in the first hours after birth
-murmurs are typically not heard
-X-ray: egg on a string appearance
-Echo.
*D-transportation: aorta is to the right of the pulmonary artery,
L-transportation: aorta is to the left of the pulmonary artery
*Ddx:
TOF
TGA
TA-truncus arteriousus
TA-tricuspid atresia
TAPVR-total anomalous pulmonary venous return
*Tr:
-prenatally: PGE1 to maintain the ductus arteriousus
-definitive surgical repair within the first week of life
