Obtain BP bilaterally
right arm blood pressure (theirs). left arm blood pressure (theirs). normal is 120/80 bilaterally
obtain apical/radial pulse
apical/radial pulse is (theirs)/(theirs), nornal is 60 to 100 bpm (there is a (apical-radial) bmp deficit)
state whether rhythm is regular or irregular
pulse is regular
inspect face and lips for color
skin and lip color appropriate for ethnicity
observe for jugular venous distention (HOB 30 degrees)
jugular venous pulsations present, no distention present with HOB at 30 degrees
inspect upper extremities for color
upper extremities color is appropriate for ethnicity bilaterally
palpate upper extremities for temperature and capillary refill
skin is warm and dry, capillary refill (theirs) bilaterally, normal is less than 2 seconds bilaterally
assess skin turgor bilaterally
turgor is resilient bilaterally
palpate carotid pulses bilaterally
carotid pulses equal bilaterally
palpate brachial pulses bilaterally, state amplitude
brachial pulses are equal, (theirs)+/4+, normal is 2+/4+ bilaterally
palpate radial pulses bilaterally, state amplitude
radial pulses are equal, (theirs)+/4+, normal is 2+/4+ bilaterally
inspect chest for masses, bulges, and lifts
no masses, bulges, or lifts noted on inspection
palpate the chest for pulsations, thrills, or heaves
no pulsations, thrills, or heaves palpated
palpate PMI and state location
PMI at 5th LICS MCL, normal is 5th LICS MCL
auscultation cardiac landmarks with diaphragm. note s1 and s2, murmurs, or rubs (aortic, pulmonic, erbs point, tricuspid, mitral)
aortic is at 2nd RICS
pulmonic is at 2nd LICS
Erbs point is at 3rd LICS
tricuspid is at 4th LICS
mitral is at 5th LICS MCL
s1 is loudest at apex, s2 is loudest at base; no s3, s4, murmurs, or rubs auscultated
