According to QRISK 3, when do you give a statin? Which statin do you usually give? At what dose?
If they have a score of >10%.
Give atorvastatin 20mg at night.
Also give to all patients with CKD or T1DM for >10 years.
Monitoring requirements for statins?
Check lipids at 3 months to aim for >40% reduction in non-HDL cholesterol.
Also check LFTs within 3 months and again at 12 months. (can have a transient mild rise in ALT and AST in first few weeks. Usually fine if rise is <3 times the upper limit of normal)
Secondary Prevention after developing Cardiovascular Disease.
The 4 As:
Aspirin - + second antiplatelet (eg clopidogrel for 12m)
Atorvastatin 80mg
Atenolol (or bisoprolol) titrated to max tolerated dose
ACE inhibitor (ramipril) titrated to max tolerated dose
