breathlessness on exertion, paroxysmal nocturnal dyspnoea, peripheral oedema, cough (nocturnal +- pink frothy sputum), orthopnoea

A

tachycardia at rest, low systolic BP, displaced apex due to LV hypertrophy, murmurs if valvular disease, increased JVP, bibasal lung crackles, peripheral oedema
