Circulation CPR discuss

A
  • Rate 100 - 120 per min
  • 5-6cm deep
  • Aim : target end-tidal CO2 >10mmHg
    Diastolic BP > 20mmHg

A
  • Adult ratio  30:2
  • Children / infants  30:2 (2 rescue breaths and then 15:2)

Defib

A
  • Shockable rhythms = ventricular tachycardia and ventricular fibrillation!
  • Any other rhythm is non-shockable
  • TBH uses biphasic defibrillators  150 J

Drugs for resus

A

consider adrenaline (incr SVR) and antiarrhythmics

**- Adrenaline = start 1 mg every 3 – 5 mins = incr SVR & CO

**- Amiodarone or lignocaine
o Usually use when there is a resistant shockable wave  resistant VT or VF
o Amiodarone (preferred)  300 mg followed by 150 mg

Amiodarone can be used in VT & SVT, unlike adenosine which can only be used in SVT!

  • Hypoxia
  • Hypovolaemia
  • Hypothermia
  • Hydrogen ion = acidosis
  • Hypo- / hyperkalaemia
  • Hypoglycaemia
  • Tension pneumothorax
  • Tamponade (cardiac)
  • Toxins
  • Trauma
  • Thrombosis (coronary)
  • Thrombosis (pulmonary)
