Difficulty Breathing
6 Components of “Inspection”
- HEENT
- Chest Shape
- Breathing Pattern
- Use of Accessory Muscles
- Skin
- Extremities
Breathing Pattern: Typical with CHF, up down up down pause, deep breath, taper off
Cheyne-Stokes
Chest Shape (2)
- Symmetry
- Rigidity
Breathing Pattern: Periods of Delay/Suspended Breathing
Apnea
Skin (4)
- Color
- Temp
- Edema
- Subcutaneous Emphysema
5 Components of Palpations
- Radial pulse
- Skin
- Tracheal position
- Spinal curvature
- Chest Expansion
Extremities (5)
- Symmetry
- Nail Bed
- Pulses
- Digital Clubbing
- Cyanosis
Breathing Pattern: Difficulty Breathing in Supine
Orthopnea
How much expansion is normal when assessing anterior/posterio chest expansion?
~2 cm gap between thumbs
Location: R middle lobe and L lingular lobe
5th rib
Breathing Pattern: Normal Breathing
Eupnea
Breathing Pattern: Increased Tidal V with Active Breathing, Increased E
Hyperapnea
Lobe:
2.5 cm above sternal border of 1st rib
R/L apex of the lung
Location: R/L Lower Lobes
8th rib
Use of Accessory Muscles (2)
- Tripod Position
- Shoulder raise with inspiration
Lobe:
7th-8th rib anteriorly and posteriorly
R/L diaphragmatic border
Define:
Cheyne-Stokes
Typical with CHF, up down up down pause, deep breath, taper off
Lobe:
b/t 2nd-4th rib
R/L Upper Lobes
Define:
Hyperapnea
Increased Tidal V with Active Breathing, Increased E
Location: R/L Upper Lobes
b/t 2nd-4th rib
Define:
Apnea
Periods of Delay/Suspended Breathing
Breathing Pattern: Difficulty Breathing
Dyspnea
Define:
Bradypnea
Slow Breathing
Lobe:
8th rib
R/L Lower Lobes
Define:
Eupnea
Normal Breathing
HEENT (3)
- Trachea position
- Sores on ears
- Eye response
Location: R/L diaphragmatic border
7th-8th rib anteriorly and posteriorly
Describe how the chest will expand with diaphragmatic vs. accessory mm breathing
D: Flare out/in
E: Move up/down
Describe how to assess anterior and posterior chest expansion
Anterior: Place your hands in a “butterfly position” on the patient’s chest just under the xiphoid process and gently but firmly pull your thumbs together at the midsternal line
Posterior: Place your hands in a “butterfly position” on the patient’s lower back (around rib 10) and gently but firmly pull your thumbs together at the midvertebral line
Breathing Pattern: Rapid/Shallow Breathing
Tachypnea
Location: R/L apex of the lung
2.5 cm above sternal border of 1st rib
Breathing Pattern: Slow Breathing
Bradypnea
Describe what percussion should sound like over
- Rib
- Intercostal space
- Atelectasis/Consolidated Alveoli
- Air Trapping
- Dull/flat
- Resonant
- Dull/flat
- Hyperresonant/tympanic
Lobe:
5th rib
R middle lobe and L lingular lobe
