How does dilated cardiomayopathy present and how do we diagnose?

A

Dyspnoea, fatigue, orthopnoea, Paroxysmal nocturnal dyspnea, ankle swelling, fluid overload and weight gain, cough

Ask about family history as can be genetic (identified SCN5A gene) and in SH ask alcohol, job

On examination: SOB at rest, thready pulse, poor superficial perfusion, elevated JVP (+/- TR waves), S3 and S4, displaed apex, ankle, sacral and abdo (acites) oedema, hepatomegally, pleural effusions and pulmonary oedema, often MR murmur
