NYHA class III or IV symptoms and resting or provoked LVOT gradient >50 despite maximum medical treatment

How is HOCM diagnosed?
A
Maximal end-diastolic wall thickness ≥ 15 mm anywhere in the LV in the absence of another cause of hypertrophy in adults.
