Cardiology updated Deck Flashcards
what class is Adenosine?
Class V antiarrhythmic
How does adenosine work?
It decreases the conduction of electrical impulse through the AV node and interrupts AV reentry pathways in paroxysmal supraventricular tachycardia
Adenosine: Onset peak effects duration half-life
onset: 20-30 secs
peak effects: 20-30 secs
duration: 30 secs
half life: 10 secs
Adenosine indication
tachyarrhythmias that refractory to vagal maneuvers (including narrow and wide tachs) and is diagnostic whether it is wide or narrow complex.
Adenosine contraindications
Irregular rhythms such as A-fib and Torsades, 2nd and 3rd-degree heart blocks, asthma, sick sinus syndrome, and ppl who have known sensitivities.
Adenosine Dosage
initial dose 6mg fast iv/io push, if more is needed next dosages are 12mg and 12mg. always followed with at least a 10 cc flush.
what class is Amiodarone?
Class III antiarrhythmic
How does Ami work
prolongs the action potential duration of ALL cardiac tissues. (affecting sodium, potassium, and calcium channel blockers)
Indications for ami
life-threatening ventricular dysrhythmias such as v-fib and v-tach, can also be used for stable afib and stable narrow complex tachycardia. it is used to control rapid ventricular rate due to accessory pathway
Ami contraindications
sinus brady, 2nd or 3rd degree av blocks, symptomatic bradycardia, or known hypersensitivity
ami interactions
warfarin, digoxin, and procainamide
Amiodarone dosage
Pulseless Vtach/Vfib: 300mg iv/io bolus followed by 150mg iv/io bolus for refractory arrythmia, max dose is 450mg
with pulses, 150mg iv/io over 10 mins
