Cardiology - Subtle STEMI - March 2018 Flashcards
1
Q
Current AHA guidelines for STEMI recognition include what STE criteria?
A
- NEW STE greater than 1 mm in two contiguous leads
2
Q
What changes to cath lab activation occur in STE in leads V2-V3
A
- NEW STE must be greater than 2 mm in men or greater than 1.5 mm in women in two contiguous leads
- What is the issue here with V2-V3???
3
Q
What is meant by “contiguous leads?”
A
Two leads that examine the same area of the heart
- Ex: II, III, aVF all look inferiorly
- Ex: V4-V5 all look laterally
4
Q
What is a “semi-STEMI?” Why is it clinically important?
A
Maximum STE of 0.7-0.9 mm.
These patients, in the HORIZONS-AMI trial, presented with a right coronary artery occlusion.
5
Q
What reciprocal changes are found in the “semi-STEMI?”
A
Typically STD greater than 1 mm.
6
Q
A small 2014 study (n=504) indicated that patients with _____ presented with coronary artery occlusion 86% of the time.
A. STE greater than 2 mm
B. STE of 1-1.5 mm
C. STE of 0.1-1 mm
A
C. STE of 0.1-1 mm
