Q

Discuss the differences between atypical (Prinzmetal) angina and typical angina.

A

Prinzmetal usually occurs in younger women, with smoking being the biggest risk factor. They usually do not have any other cardiovascular risk factors. Attacks classically occur in the middle of the night. In Prinzmetal, attacks show transient ST-segment elevations that disappear when the attacks resolve. It is a vasospastic disease similar to Raynaud’s. Treatment usually involves CCBs or nitrates.

Typical angina . It is an atherosclerotic narrowing, as seen in claudication. EKG findings include ST depressions. Patients are usually older and have multiple cardiovascular risk factors.
