Angina
Ischemic heart disease manifestation
Chest pain secondary to coronary artery narrowing or spasm
No monocyte necrosis
Stable: secondary to atherosclerosis, exertion chest pain, ST depression, resolves with rest or nitroglycerin
Variant (Prinzmetal): secondary to coronary artery spasm, transient ST elevation, triggers include tobacco, cocaine, triptans; treat with Ca channel blockers, nitrates, and smoking cessation
Unstable: thrombosis w incomplete coronary artery occlusion, +/- ST depression and/or T-wave inversion, no cardiac biomarker elevation, ^ in frequency or intensity of chest pain or chest pain at rest
Myocardial Infarction
Most often acute thrombosis due to rupture of coronary artery atherosclerotic plaque and complete occlusion of a coronary artery
Necrosis of cardiac myocytes
^ cardiac biomarkers (CK-MB good for re-infarcts, troponins)
Symptoms: diaphoresis, nausea, vomiting, severe retrosternal pain, pain in left arm and/or jaw, dyspnea, fatigue; not relieved by nitro
Usually involves LV (RV and atria generally spared)
