  1. Severe hypovolemia (hemorrhage or severe dehydration)
  2. Altered cardiac pump function/critical reduction in myocardial contractility (MI, major arrhythmia, severe valvular disease)
  3. Major obstruction (cardiac tamponade, PE, tension pneumothorax)
