Epinephrine
Indications: cardiac arrest, sympto brady
Contra: N/A
Side effects: HTN, tachycardia
Doses: Arrest- 1mg of 1:10,000 every 3-5 min
Brady- 1mg/250mL of 1:1,000 @ 2-10 mcg/min
Dopamine
Indications: hemodynamically significant hypotension without hypovolemia
Contra: hypovolemia, tachyarrhythmias
Side effects: HTN, tachycardia
Doses: 2-20mcg/kg/min IV infusion
=D.O. x Pt. Kg x gtts set / concentration
Amiodarone
Indications: V-Fib., pulseless V-Tach, some SVT’s
Contra: sinus brady, 2nd or 3rd degree block
Side effects: hypotension, bradycardia
Doses: arrest-300mg, repeat at 150mg x1
V-tach-150mg/100mL over 10 min
Maintenance-150mg/100mL @ 1mg/min
Lidocaine
Indications: alternate to amio in arrest and V-tach with a pulse
Contra: allergy, 2nd or 3rd degree block
Side effects: bradycardia, hypotension
Doses: arrest-1-1.5mg/kg not more than 3mg/kg
V-tach-0.5-0.75mg/kg IV
Maintenance-1-4mg/min of 2g/500mL (4:1 concentration) use clock method
Mag Sulfate
Indications: eclampsia, torsades de pointes
Contra: hypotension, bradycardia
Side effects: CNS depression, hypotension
Doses: 1-2g/100mL IV over 5-60 minutes
Morphine
Indications: severe CHF, pulmonary edema, chest pain associated with myocardial infarction
Contra: head injury, depressed respiratory drive, hypotension
Side effects: decreased loc and respiratory drive, hypotension
Doses: 2-4mg IV push not more than 10mg
Atropine
Indications: symptomatic brady
Contra: tachycardia
Side effects: paradoxical Brady if pushed to slow
Doses: arrest-1mg IV not more than 3mg
0.5mg IV 3-5 min, not exceeding 3mg
Adenosine
Indications: SVT, PSVT
Contra: sick sinus, 2nd or 3rd degree block
Side effect: flushing, SOB, chest pain
Doses: 6mg rapid IV followed by 20cc flush, repeat at 12mg x2
Sodium bicarbonate
Indications: metabolic acidosis
Contra: alkalosis
Side effects: alkalosis
Doses: 1meq/kg IV, repeat at half original dose in 10 min
Narcan
Indications: opiate overdose
Contra: N/A
Side effects: tachycardia, HTN, N/V
Doses: 0.4-0.8mg IV, repeat as needed not more than 2mg
Aspirin
Indications: new onset chest pain, suggestive of MI
Contra: NSAID allergy, active ulcers
Side effects: GI bleeding, N/V
Doses: 324-325mg PO
Nitro
Indications: acute chest pain, ischemic chest pain
Contra: hypotension, previous use of ED Rx, inferior MI
Side effects: headache, hypotension, flushing
Doses: 0.4mg SL, repeat x3 3-5min
Lopressor
Indications: suspected ACS with heart rate over 80 and systolic over 120
Contra: heart rate less than 45, systolic less than 100, 2nd or 3rd degree block
Side effects: headache, dizziness, dyspnea
Doses: 5mg slow IV, repeat x3 every 2 min not more than 15mg
Versed
Indications: conscious sedation when cardioverting
Contra: EtOH, overdose, depressed vital signs
Side effects: N/V, respiratory depression
Doses: 2-5mg slow IV
Procainamide
Indications: stable and wide complex tachycardia of unknown origin
Contra: torsades de pointe, 2nd or 3rd degree block
Side effects: widening PR, QRS, and QT intervals, bradycardia, and N/V
Doses: 20-50mg/min IV not more than 17mg/kg or 1g. Maintenance: 1-4mg/min.
**stop maintenance at 1g if hypotension or 50% widening QRS presents
