Epinephrine

A

Indications: cardiac arrest, sympto brady

Contra: N/A

Side effects: HTN, tachycardia

Doses: Arrest- 1mg of 1:10,000 every 3-5 min
Brady- 1mg/250mL of 1:1,000 @ 2-10 mcg/min

A

Indications: hemodynamically significant hypotension without hypovolemia

Contra: hypovolemia, tachyarrhythmias

Side effects: HTN, tachycardia

Doses: 2-20mcg/kg/min IV infusion

A

Indications: V-Fib., pulseless V-Tach, some SVT’s

Contra: sinus brady, 2nd or 3rd degree block

Side effects: hypotension, bradycardia

Doses: arrest-300mg, repeat at 150mg x1
V-tach-150mg/100mL over 10 min
Maintenance-150mg/100mL @ 1mg/min

Lidocaine

A

Indications: alternate to amio in arrest and V-tach with a pulse

Contra: allergy, 2nd or 3rd degree block

Side effects: bradycardia, hypotension

Doses: arrest-1-1.5mg/kg not more than 3mg/kg
V-tach-0.5-0.75mg/kg IV
Maintenance-1-4mg/min of 2g/500mL (4:1 concentration) use clock method

Side effects: headache, dizziness, dyspnea

Doses: 5mg slow IV, repeat x3 every 2 min not more than 15mg

A

Indications: stable and wide complex tachycardia of unknown origin

Contra: torsades de pointe, 2nd or 3rd degree block

Side effects: widening PR, QRS, and QT intervals, bradycardia, and N/V

Doses: 20-50mg/min IV not more than 17mg/kg or 1g. Maintenance: 1-4mg/min.

**stop maintenance at 1g if hypotension or 50% widening QRS presents
