Indicated by ST depression and angina during stress test. Diagnosis of exclusion
Prinzmetal angina
Caused by coronary artery vasospasm
Cardiac syndrome X
Caused by microvascular dysfunction
Causes of resting ST elevation
MI, pericarditis, left bundle branch block, left ventricular hypertrophy, coronary artery spasm, LV aneurysm, ventricular pacing, early repolarization
Hibernating myocardium
Chronically underperfused myocardium. Perfusion may restore normal contractility
Reperfusion injury
Occurs 1 hour after perfusion with further irreversible myocardial damage
Stunned myocardium
Caused by acute ischemia. Function may be restored 7-10 days after perfusion
