Cardiac Flashcards
Retrograde cardioplegia is good for …
severe AI, severe CAD, complex or abnormal aortic root surgeries
limitation of retrograde cardioplegia
doesn’t get to RCA or anterior free wall territory very well
when do we use deep hypothermic circulatory arrest?
pulmonary thromboembolectomy or complex aortic arch work (15-22 degrees, complete arrest of blood flow)
when would we use ventricular fibrillation as protection?
brief aortic cross clamping - don’t want to increase myocardial stress against that afterload.
what happens autonomically to donor heart?
no parasympathetic - denervated from cutting autonomic cardiac plexus. Will respond to direct actors (epi, dobutamine) but not indirect or PSNS (atropine, ephedrine)
(resting HR 90-110)
